Mechanism:
Insulin stimulation effect varies according to sulfonylurea, and it is due to structural changes which would modify binding affinity to its receptor localized in the plasma membrane. The Sulfonylurea Receptor (SUR) belongs to the ATP-Binding Cassette family as a 140-170 kDa membrane protein. The SUR is structurally related to another protein called Inwardly Rectifying Potassium Channel and together they constitute the KATP channel, one of the major metabolic sensors in pancreatic beta cells.
At the molecular level, metformin inhibits the mitochondrial respiratory chain in the liver, leading to activation of AMPK, enhancing insulin sensitivity (via effects on fat metabolism) and lowering cAMP, thus reducing the expression of gluconeogenic enzymes.
Indication:
It is indicated for the treatment of Type 2 diabetes
Dosage:
1 tablet once daily or to be taken in a dose and duration as directed by the Physician.
